All applications for licenses shall be in writing, on a form provided by the City License Clerk, and shall be submitted to the License Clerk or other designated representative along with the required license fee and/or tax. The application shall contain all information necessary to determine the kind of license desired; the place where such business, calling, trade or profession is to be carried on; and any other matter or thing which may be required by the License Clerk and necessary for the purpose of enabling the License Clerk to fix the amount of the licensee fee and/or tax.
