Every license certificate shall bear on its face:

    A.    The name of the person, firm or corporation to whom the license certificate has been issued;

    B.    The amount of the license fee and/or tax paid to the City;

    C.    The kind of license, with the dates of its commencement and expiration;

    D.    Address of the place where such business, calling, trade or profession is carried on. (Prior code § 11-02-012)
