Chapter 5.38 TEMPORARY SALES EVENTS
Section 5.38.030 License--Application--Contents.
A. All applicants for licenses required
by this chapter shall file a written application with the
City License Clerk. Such application shall be signed by the applicant, or an authorized
representative. Application shall be made on forms available in the office of the City Licensing
Clerk, and shall include the following:
1. The name and address of the applicant;
2. The name of the individual having
management authority or supervision of the applicant's
business during the time that it is proposed to be carried on in the City;
3. Address of business;
4. The permanent address of such individual;
5. The capacity in which such individual
will act (Manager or owner);
6. If a corporation, the state in
which incorporated;
7. The time period(s) during which
it is proposed to carry on the applicant's business;
8. The nature and character of the
goods or services to be offered for sale or delivery;
9. The nature of the advertising proposed
to be done for the business;
10. Whether or not the applicant or
the individual identified in this subsection has been
convicted of any crime or misdemeanor, and if so, the nature of each offense and the penalty
assessed for each offense;
11. A list of all participants, including
their names and addresses, location of the sales
event(s), and dates of commencement and termination of the sales event(s).
B. Applicants who propose to handle
foodstuff shall also attach to their application a
statement from the Health Inspector, dated not more than ten (10) days prior to the date of
application, that the applicant and any products to be sold meet all the related health
